Any bleeding around the elbow may point towards an open fracture and needs to be assessed. The arm may be swollen, bruised, and deformed. The affected limb must be thoroughly assessed for the integrity of the soft tissue envelope and the distal neurovascular status. Any other injuries must also be carefully evaluated and documented. ![]() Young children may be reluctant to use the affected limb. The child or carer may give a history of pain, swelling, or deformity of the affected limb. There may be a history of a fall on an outstretched arm, causing an extension-type fracture or a fall on a flexed elbow. Although rare, a non-accidental injury must be excluded, accounting for <0.5% of the fractures. On the other hand, posterolateral displacement tends to rupture the medial periosteum, and the reduction is therefore performed in supination. Due to this, the reduction is performed in pronation which tensions the medial periosteum and prevents varus malalignment. In posteromedial displacement of the distal fragment, the medial periosteum is intact. The mechanism of injury and the resultant displacement of the fracture also guides the reduction maneuver performed. Flexion-type fractures occur with a fall on a flexed elbow but are much less common. This is the mechanism of injury for extension-type fractures, which account for up to 95% of supracondylar fractures. ![]() The posterior cortex and periosteum may be intact. The bone fractures at the weakest point, and the anterior humeral cortex fails in tension. The olecranon process engages the olecranon fossa producing an extensive force on the distal humerus. Most fractures occur when the child falls on an outstretched arm causing hyperextension of the elbow. In the axial plane, it is dumbbell shaped owing to the thin plate of the bone in the middle, at the site of the olecranon fossa and coronoid fossa. The shape of the distal humerus is roughly triangular in the coronal plane, with the base composed of the lateral epicondyle, capitellum, trochlea, and medial epicondyle. The incidence of open supracondylar fractures is around 1%. These injuries often occur in the non-dominant upper extremity. Flexion-type fractures are more prevalent in older children. Also, it has been reported that these fractures are more common on weekends and during summer. Ligamentous laxity in the pediatric age group causes hyper-extension of the elbow and contributes to causing extension-type fractures. Extension-type fractures are much more common than flexion-type fractures and occur as a result of a fall on an outstretched hand. However, a recent epidemiological study found no significant difference between the genders. Some studies describe an increased prevalence among males than females, while others describe a higher prevalence in females. The median age is approximately six years, and the incidence gradually decreases with age. ![]() They most frequently occur in children aged 5 to 7 years. The supracondylar region of the distal humerus is the most commonly fractured area around the elbow in children. On the other hand, some patients who develop a malunion may have a satisfactory function in the long term. Inadequate reduction and fixation can also lead to malunion and deformity. These injuries can be significant due to the risks of neurological damage, vascular injury, and compartment syndrome. The modified Gartland classification is often used to describe fracture patterns and guide treatment. Despite evolutions in the evaluation and treatment over the years, these fractures still pose considerable challenges to orthopedic surgeons. Gartland described that even expert trauma surgeons are anxious and apprehensive while managing supracondylar humerus fractures. Supracondylar fractures of the humerus account for up to 18% of pediatric fractures and 60% of elbow fractures. Children often utilize an outstretched arm as a protective mechanism when falling, creating a high incidence of fractures about the elbow. Elbow fractures are more common in the pediatric population than in the adult population. ![]() ![]() Step inside the 2005 Dodge Magnum SXT, and you’ll be greeted by luxurious Graystone leather upholstery, adorning the heated and power-adjustable front bucket seats. ![]() (Go to Mouse Properties and click the tab for Pointer Options.) In some cases, there may actually be something wrong with the mousepad, such as a loose connection. Others say they have been helped by unchecking the option that says "Hide mousepointer while typing". Some people report that their pointer-jumping problem was solved by installing the latest mousepad driver. There's also a free, open source utility called Touchfreeze for Windows NT, 2000 and XP, which "automatically disables touchpad while you are typing text. That will tell you whether or not the mousepad is the culprit. Another is to disable the mousepad in the control software and use an external mouse. If there isn't an on/off button by the pad, read the manual for the command: it might be something like Fn+F3, or Fn+F9. One is to turn the mousepad off while typing. If disabling "tap to click" doesn't remove the problem, there are slightly more extreme solutions. In fact, some seem to be so sensitive they could be affected by people hitting nearby keys such as the space-bar, though I have no concrete evidence for this. Either my skin is less sensitive than it used to be, or mousepads are far more sensitive than they used to be, or both. However, I was making enough contact to affect the mouse pointer and move the cursor. I had been certain that I wasn't touching the mousepad, and not brushing it with my palm. ![]() You can also adjust its sensitivity to PalmTracking. On my Asus U元0A, this meant right-clicking on the Elan Smart-Pad icon in the Systray (bottom right), selecting the Elan tab in Mouse Properties, clicking Options and then Tapping. I did solve the problem in the end, by going to the touchpad controller and turning off (unchecking) the feature often known as "tap to click". Neither Microsoft nor the manufacturer could help me turn this feature off, probably because it didn't exist. The mouse seemed to be clicking itself to select actions on a mouse-hover, which could be disastrous. However, I did run into it recently with a couple of netbooks running Windows XP, and a thin-and-light Windows 7 portable that I bought myself. ![]() I'd not run into the problem because, for decades, I've used Toshiba Portégé and IBM ThinkPad laptops with pointing sticks (TrackPoint, PointStick etc) instead of mousepads. To identify it as a fault, we really want to know the make and model of the mousepad, the version number of the software driver, and similar details. ![]() Typically people mention the brand of PC and the operating system, which makes the problem appear to be completely random and therefore beyond rational solution. This is obviously a simple arithmetic dyscalcalcution in the Remote Desktop App which should be very easy to find. This problem comes up quite often on the net, usually on PCs but sometimes on Macs, but almost never with any useful information. This behaviour totally distracts my concentration and drops my productivity to less than 50% of the normal. But it gets worse, the newer the mouse is, maybe due to higher resolution. This happens when the mouse cursor changes from arrow to double-arrow (resizer) style. Sometimes it flips back to the right positions, sometimes it does not. When running a remote desktop an trying to resize a window or a gui element, the mouse cursor very often jumps to a position exactly one cursor width and height right below where the cursor should actually be. ![]() ![]() ![]() FedEx Priority Overnight: The delivery to houses and rural locations takes place on the following working day between 10:30 a.m.FedEx Express Saver: Delivery takes place on the third business day, by 4:30 p.m.Delivery and pickup are optional on Saturday and are available at some locations only.
